To Whom It May Concern:

This letter serves as my formal notification of intent to vacate the premises shortly after the conclusion of my lease period.

Shortly after my arrival at Hidden Palms Apartments, I began to experience upper respiratory problems. Since that time, I have seen the doctor on several occasions for antibiotics and an explanation for why I have recently been having this problem. Recently, the roof of the laundry room began leaking. Upon closer inspection, I realized the roof and walls were covered in mold. This mold has visibly extended into the adjacent coat rack closet by the front entrance, has ruined two coats, and the mold stains have appeared on the other side of the wall facing the living room. In light of my recent respiratory problems, likely brought on by the presence of mold as often documented by health professionals, living in this apartment has become a health hazard.

My lease expires on May 1, 2004. Due to the mold and its likely cause of my recent respiratory problems, this letter serves as my formal notice that I will be vacating the premises within two weeks of the end of my lease term. Because of the dangerous nature of mold and the clear health hazard it poses to myself and others, I request that no penalty or fees be attached to my departure within the notice period.
